Mechanism/Pathway Tumor-necrosis factor-α (TNFα) is a major proinflammatory cytokine secreted mainly by macrophages and the TH1 subset of T-helper cells. Increased expression of TNFα has been shown to be associated with clinical activity in relapsing remitting MS and the development of chronic progressive disease. TNFα augments the immune response by induction of MHC class II antigens, adhesion molecule expression, and blood-brain barrier dysfunction. Finally, TNFα has been shown directly to cause oligodendrocyte damage and demyelination in vitro.
